Only then / Only after 〜 / Only when 〜 が文頭に出ると、主節が疑問文と同じ語順（did S V ／ was S 〜 など）の倒置になります（Only then did I realize ＝ そのとき初めて分かった）。

倒置は元の SV に戻し、Only 〜 は「〜して初めて」
